With the Ascension Personalized Care plan, you will have access to our care management team who is here to help you navigate the complex world of healthcare. Our care management team is made up of registered nurses, licensed clinical social workers and community health care workers who are here to listen and help you meet your healthcare goals. What is an HRA?HRA stands for “health risk assessment.” A health risk assessment is a screening tool that helps identify and understand health risks. It helps your doctor or care team monitor your health status over time. An HRA includes a questionnaire, an assessment of health status, and personalized feedback about actions that can be taken to reduce risks, maintain health, and prevent disease. While it is not required, it is very beneficial for the member to help understand their healthcare needs.

What is preventive care? What is covered?Ascension Personalized Care pays for 100% of certain preventive care services. These services have no out-of-pocket cost for you. Preventive care services help you get and stay healthy and include screenings, immunizations, and checkups. There are some exceptions, and it is important to read the Ascension Personalized Care preventive care services guidelines. This guide will tell you what is covered as preventive care and when you may be charged additional costs based on the kind of services received.
