How do I reset my password?

To reset your password, visit member.ascensionpersonalizedcare.com. You will then select "Forgot Password" and provide the email address you used when creating your account. You will receive an email that will allow you to reset your password.
How do I create an online member account?

To create an account for your Ascension Personalized Care member portal, visit member.ascensionpersonalizedcare.com. Click the "Sign Up" button to begin. Next, you will be asked to enter your email address and verify this address by providing a code sent to your inbox. After validating your email and choosing a password, you will be asked to secure your account with a two-factor authentication.
