What are network doctors?Network doctors are a list of healthcare professionals who are contracted with US Health and Life Insurance Company, the insurer that offers Ascension Personalized Care, and provide medical care to our members.

How do I get care from out-of-network doctors?
You may visit a doctor outside of the network. However, because the doctor is not within the Ascension Personalized Care network, the amount billed for services will be higher. If you prefer to be seen by an out-of-network provider, you will be responsible for paying the difference between the amount billed and your plan's approved amount. -
What is a PCP and how do I choose one?A PCP is a primary care provider. A PCP can be a doctor, nurse practitioner, clinical nurse specialist or physician assistant. These providers help patients with a wide range of healthcare services. To choose a PCP, click Find a Doctor and you will be able to browse doctors in your area that are accepting new patients.

To find a PCP in your area, click Find a Doctor. You will be able to browse doctors in your area that are accepting new patients. -
How do I change my PCP (primary care provider)?
Changing your PCP is simple. Just find a doctor you are interested in and schedule an appointment. If your doctor is within the Ascension network, your medical records will automatically be available to the new doctor.
