Notification about Ascension Personalized Care insurance plans for 2025

Important:

Your health coverage is ending. Take action by December 15, 2024, or you may not have health coverage in January 2025.

Thank you for choosing Ascension Personalzed Care ("APC") for your health care needs. We're here to help you prepare for Open Enrollment.

Important Notice:

Starting 2025, APC will no longer offer curent health coverage in the Health Insurance Marketplace. The last day of your current Marketplace coverage is December 31, 2024. Read this notice carefully and review your options. Choose a different plan by December 15, 2024 to avoid a gap in your coverage.

What you need to do:

Review your coverage options and pick a different plan. If you don't have health coverage, you'll have to pay for all of your health care.

1. Update your Marketplace application by December 15, 2024

Review and, if necessary, update your Marketplace application to make sure the information is still current and correct, and to see if you may qualify for more or less financial help in 2025 than you are getting now. This may result in a lower monthly premium payment or lower out-of-pocket costs (like deductibles, copayments and coinsurance). Plus, you can help avoid paying money back when you file your taxes.

2. Choose a different plan. Here are some ways to look at other plans and enroll:

Visit healthcare.gov to find other Marketplace plans. Compare plans to save money and find a plan that best meets your needs and budget. Select the Plan Name and ID of the plan you want to enroll in. If you don't enroll in a plan on your own, the Marketplace may automatically enroll you in a plan it picks for you. Also, if you currently have a Bronze category plan and qualify for extra savings, the Marketplace may enroll you in a Silver plan so you'll get the most help to lower your costs in 2025. If you choose a plan between November 1, 2024 and December 15, 2024, the new plan will start on January 1, 2025. If you choose a plan between December 16, 2024 and January 15, 2025, the new plan will start February 1, 2025.

Note: If you received financial help in 2024 to lower your monthly premium, you'll have to "reconcile" using IRS Form 8962 when you file your federal taxes. This means you'll compare the amount of premium tax credit you got in advance with the amount you actually qualify for based on your final household income and eligiblity information. If the amounts are different, it may change the amount you owe or get back when you file your taxes. Are Ascension Personalized Care plans going away in every county?

Yes, Ascension Personalized Care will no longer be offering plans on-exchange or off-exchange in any state, which is why you are receiving this notice.

When will coverage end?

Your coverage with your current plan, APC, will end on December 31, 2024. After that, on-exchange members will be mapped to a different on-exchange plan for 2025. You can also select a different plan from healthcare.gov. If you bought your plan off-exchange, you must select a new plan to avoid losing coverage.

Can you make an exception for me?

No. We cannot make any exceptions for any members.

When can I enroll in new Affordable Care Act Individual and Family Coverage for 2025?

The Health Insurance Marketplace Open Enrollment Period for 2025 is November 1, 2024 through December 15, 2024 for a January 1, 2025 effective date. During the Open Enrollment Period you have the opportunity to compare plans and enroll in a new plan for 2025.

If I want to find a new plan, how do I go about find a new Affordable Care Act Individual and Family plan?

There are tools and resources at healthcare.gov to assist you in choosing a new plan.

Is there a way for me to tell if my current Ascension Personalized Care doctors are coveraged as I consider new plans for 2025?

The best way to determine this information is to ask your healthcare doctor whether they are in-network with the insurance plans you are considering. If you prefer to research yourself, the tools on healthcare.gov can help you find an alternate plan or direct you to local resources to help with your search. You can also see whether doctors are in-network with the health plans you are considering.

How can I ensure that all my information rolls over to my new plan for 2025?

Your new plan will help you transition your current care plans and prescriptions to your new benefit in 2025. You should call your new plan at the beginning of the year to make sure they have all of your relevant information and have all of your information correct.

Can I call Ascension Personalized Care to get more information?

If you have questions or concerns and would like to contact Ascension Personalized Care directly, please call our Uniquely Ascension Service Center at 833-600-1311 (TTY: 586-693-1214). We are available Monday through Friday, 8:00 a.m. to 6:00 p.m. EST. Please keep in mind that the Service Center cannot help you shop for a new plan or answer any questions about your new plan. Someone at healthcare.gov can be contacted at 800-318-2596 (TTY: 855-889-4325). Hours of operation are 24 hours a day, seven days a week (except holidays).

Will the Exchange be moving Ascension Personalized Care members into new plans for 2025?

The Exchange will be passively moving impacted Ascension Personalized Care members to new health plans for 2025. You will be mapped to a different on-exchange version of your current plan through this process. Members can change their plan any time during the Open Enrollment Period, and you should contact your broker or visit healthcare.gov to review your plan choices for 2025. If you enrolled off-exchange, you must actively select a new plan for 2025.
